Overview
DO YOU WANT TO HELP FELLOW SURVIVORS OF COLON AND RECTAL CANCER? Join the Survivorship Plan HEalth REcord (SPHERE) study to help us deliver survivorship care tools and resources designed for people like you!
We are looking for volunteers, 18 years of age or older, who have recently undergone surgery for stage 1, 2, or 3 colorectal cancer, to help us evaluate two different ways of delivering survivorship or follow-up care information to Colorectal
Cancer (CRC) patients.
Let us offer you survivorship care tools and resources, at no cost, to help you self-manage your care and help your community by telling us what you think about these resources.
Just complete 4 surveys in the next year—no travel is necessary, we will remind you about the surveys, and you will also receive gift cards for sharing your experience with us!

Description
What Will Happen During Study Participation?
You will be randomly assigned to one of two approaches to delivering information about cancer survivorship.
The topics that both approaches will cover include the following:
- Follow-up Care: guidance about appropriate follow-up or surveillance care
- Prevention: guidance about preventive behaviors and cancer screening
- Support Communities: local and national information resources and support groups
- Symptoms: guidance about how to self-manage your symptoms
A study team member from the IU School of Medicine/Regenstrief Institute will reach out to schedule a time to familiarize you with the study. This meeting usually takes around 30 minutes and can be done over the phone or video-- we will help you access your surveys and introduce you to your care plan.
Additional Information
You can expect to be enrolled in the study for 12 months; we will reach out to you when it's time for your study surveys.
We value your willingness to share your experiences to help your community and we want to give you up to $60 in gift cards for completing our study surveys!
Eligibility
You may be eligible for this study if you meet the following criteria:

colon cancer, rectal cancer, colorectal cancer

Age: 18 Years

Gender: All
Inclusion Criteria:
- Diagnosis of colon or rectal cancer, Stages I, II, or III and have undergone surgery for CRC within the past 12 months
- English-speaking
- Must have regular (at least weekly) access to an internet connection, whetherhome or elsewhere (family or friend)
Exclusion Criteria:
- Recurrence of colorectal cancer
- Dementia or other severe cognitive disability
- Schizophrenia or other psychosis
- Currently in hospice care
